Absorption of lactose and its digestion products i the normal and malnourished Ugandan.

چکیده
Primary or specific lactase deficiency is very common in healthy members of the Baganda and other Bantu tribes of Uganda after the first three or four years of life (Cook and Kajubi, 1966; Cook, 1967). Secondary lactase deficiency has been recorded in some cases of kwashiorkor (Bowie, Brinkman, and Hansen, 1965; Stanfield, Hutt, and Tunnicliffe, 1965), where there is mucosal damage in the proximal jejunum. The functional importance of the lactase deficiency in both of these situations is not yet clear. The lactose-tolerance test has been widely used as a screening test for lactase deficiency and it is important to establish its true value. Several investigators have found a good correlation between the lactase level in specimens of jejunal mucosa and the maximum rise in blood glucose after lactose (Dunphy, Littman, Hammond, Forstner, Dahlqvist, and Crane, 1965; McMichael, Webb, and Dawson, 1965, 1966; Peternel, 1965; Cook and Kajubi, 1966; Welsh, 1966). Others have not found such a good association (Friedland, 1965; Newcomer and McGill, 1966a). The present study was undertaken to assess the relationship between the rise in capillary blood glucose and percentage absorption of lactose digestion products from the jejunum after oral lactose. By an intubation technique the amount of unabsorbed lactose in the jejunum was measured. In a few subjects jejunal disaccharidase levels were also estimated. Two groups were studied: a series of relatively normal Ugandan hospital patients, and a group of children with mild to moderately severe kwashiorkor.

Comparison of three methods used in assessment of carbohydrate absorption in malnourished children.
James, W. P. T. (1972). Archives of Disease in Childhood, 47, 531. Comparison of three methods used in assessment of carbohydrate absorption in malnourished children. The results of oral lactose and sucrose tolerance tests were compared with jejunal disaccharidase activities and the absorption rates of lactose and sucrose from a perfused segment of jejunum in 8 malnourished children before and ...
متن کاملSugar absorption and intestinal motility in children when malnourished and after treatment.
1. The ability of malnourished children to absorb lactose, sucrose and glucose was tested by perfusing the jejunum. Intestinal motility was measured simultaneously in a perfused segment by a dye dilution technique. These tests were repeated on the same children after 6-16 weeks of treatment. 2. There was a significant correlation between the rate of hydrolysis of disaccharide perfusing the jeju...
متن کاملLactose digestion by human jejunal biopsies: the relationship between hydrolysis and absorption.
The relationship between lactose hydrolysis and absorption of released glucose was investigated by determining the kinetics of lactose digestion by jejunal biopsies incubated in vitro. Lactase activity in intact biopsies correlated with conventional assay of tissue homogenates (r = 0.85, p less than 0.001), and glucose uptake from 28 mM lactose was directly proportional to lactase activity (r =...
متن کاملEffect of lactose hydrolysis on calcium absorption during duodenal milk perfusion.
A multi-lumen intubation system was used to study the absorption of calcium, glucose and galactose in 13 human subjects. The intubation was placed between the duodenum abdomen and proximal jejunum and the subjects were perfused with milk and lactase-supplemented milk. Lactose disappearance over a 20 cm length of intestine was used as the index of lactase activity. The subjects were assigned to ...
متن کاملSugar intolerance in malnourished infants and children.
Of 100 malnourished Indian infants and children 50 showed a stool pH below 6. Carbohydrate-loading tests demonstrated intolerance to lactose in 39, to sucrose in 25, and to maltose in 15. D-Xylose absorption was reduced in 16. Steatorrhoea was detected in 7 of 18 children tested. Nutritional recovery reversed the abnormalities in all except four, in whom a primary enzymic deficiency was conside...
